Summary:
The Outreach Work supports the Patient Centered Medical Home (PCMH) model by working with community members to increase access to affordable health care. The Outreach Worker conducts outreach and educational activities with an emphasis on assisting community members in enrollment in the Kentucky Health Benefits Exchange. The Outreach Worker will be an integral member of the patient engagement and support team. The Outreach Worker position is funded through grant funds and continuation is contingent on these funds.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
GENERAL DUTIES:
The Outreach Worker must be able to travel locally and regionally as well as attend conferences, training and other events as required in order to maintain proficiency in fulfilling the responsibilities of the position. Reliable transportation is essential to performing the duties of this position.
QUALIFICATION REQUIREMENTS:

SKILLS:
Minimum of Associates Degree in Human Services; Bachelor’s in Social Work preferred

The employee must occasionally lift and /or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Conduct outreach activities at local and regional events to educate community members
- Participate in trainings provided by the state related to enrollment in the Kentucky Health Benefits Exchange (KHBE) and pass appropriate certification exams.
- Conduct training for health center staff and other agency staff as requested
- Assist qualified persons in enrollment and / or re-enrollment in the KHBE in-person and by phone.
- Accept referrals for assistance with enrollment and re-enrollment.
- Maintain all required documentation and submit monthly reports detailing outreach and enrollment efforts
- Travel to consumer locations to assist with enrollment activities (i.e., home visits, other agencies, churches, etc.)
- Assist consumers in the appeals process when needed
- Maintain a high level of consumer satisfaction
- Assist consumers with referrals to health care resources within their community

- Team members will participate in the review and evaluation processes of practice performance and help to identify opportunities for improvement.
- Team members will participate in Grace Health's advocacy program.

- Excellent interpersonal skills, including strong verbal and written communication skills
- Experience with public speaking and presentations
- Competent in use of software including Excel and database management
- Excellent organizational skills
- Ability to work in a team-oriented environment
- Ability to manage multiple assignments independently
- Knowledge of community needs and initiatives and/or experience working in a community health setting
- Valid driver’s license and access to reliable vehicle
- Knowledge of local and regional resources for low-income and at-risk populations
