| Summary: The Clinical Administrative Assistant/LPN is a care team member with unique responsibilities that indirectly touch all aspects of patient care. The Clinical Administrative Assistant/LPN embraces a team-based approach and is comfortable working as part of the patient care team and serving as a liaison to the CMO. The functions of this position include care coordination, case management, patient navigation, organization of provider meetings/trainings and other duties as directed by the CMO. |

EDUCATION and/or EXPERIENCE: Current licensure as Licensed Practical Nurse is required. Experience in the primary care setting is preferred. |
